Here’s what you’ll need to enroll your child:
1. Application Form

2. Proof of Age/Identity (choose one)
- Child’s birth certificate
- Passport
- Baptismal certificate (with date of birth)
3. Proof of Arizona Residency (choose one)
- Utility bill (water, electric, gas)
- Lease agreement or mortgage statement
- Driver’s license or ID with address
- Affidavit of Shared Residence (if applicable)
4. Immunization Records or Exemption Form
- Submit a copy of current immunization record
- OR complete the ADHS exemption form (available at our office)
5. Previous School Records (if applicable)
- Report cards or transcripts
- Withdrawal form from previous school
- Special education or IEP/504 documents, if any
6. ESA Scholarship Confirmation (if applicable)
- If applying under Arizona’s Empowerment Scholarship Account, please submit your ESA award letter or current documentation.
